It's the Hunter Altitude fan. We got it at Home Depot, but it doesn't look like they carry it anymore. Maybe you can track it down somewhere else or directly from Hunter.
My husband and I love your blog. Can you tell us more info about this fan? It's so hard to find attractive modern fan/light combos.
